Drainage is essential, discuss with respiratory team:

• Piperacillin/tazobactam  IV 4.5g tds plus Vancomycin IV 1g bd (adjust dose if >65 yrs or renal impairment; monitor levels)

If allergic to penicillins:

• Discuss with microbiology

• Review antibiotics with microbiology results

• Discuss with microbiology for oral continuation therapy
