Antibiotic Guideline for the Treatment of Endocarditis:

Endocarditis when allergic to penicillins:

++All therapy and investigation should be discussed with Microbiology / Infectious Diseases.++

Viridans streptococci, Streptococcus bovis,

Fully sensitive to penicillin (MIC < 0.1 mg/l)

Vancomycin IV 1 g BD (adjust dose if >65 yrs or renal impairment; monitor levels)

if native valve for four to six weeks depending on clinical response
or if prosthetic heart valve six weeks

Reduced sensitivity to penicillin (MIC > 0.1 mg/l)

Vancomycin IV 1 g BD (adjust dose if >65 yrs or renal impairment; monitor levels) (for four to six weeks)* plus Gentamicin IV 80 mg BD (monitor levels) for two weeks.

* If native valve for four to six weeks depending on clinical response
or, if prosthetic heart valve, six weeks treatment.
