Antibiotic Guideline for the Treatment of Endocarditis:

++All therapy and investigation should be discussed with Microbiology / Infectious Diseases.++

- If penicillin allergic click here.

Enterococci, Penicillin resistant (MIC > 0.5mg/L) S. bovis/viridans strep, nutritionally variant streptococci

Gentamicin sensitive or low level resistant (MIC < 100 mg/l)

Amoxicillin IV 2 g every four hours plus Gentamicin IV 1.5mg/kg BD (monitor levels) , (click here for duration of therapy)
(Alternatives, on the advice of a microbiologist)

Gentamicin sensitive or low level resistant (MIC < 100 mg/l) but penicillin resistant (MIC >8mg/l)

Vancomycin IV 1 g BD (adjust dose if >65 yrs or renal impairment; monitor levels) (Alternatives, on the advice of a microbiologist) plus Gentamicin IV 1.5mg/kg BD (click here for duration of therapy)

Gentamicin highly resistant (MIC > 2000 mg/l)

Amoxicillin IV 2 g every four hours (Streptomycin can be given if strain is sensitive following discussion with microbiology) for duration discuss with microbiology.
