Antibiotic Guideline for the Treatment of Endocarditis:

Viridans streptococci and Streptococcus bovis

++All therapy and investigation should be discussed with Microbiology / Infectious Diseases.++

- If penicillin allergic click here.

Fully sensitive to penicillin (MIC < 0.1 mg/l)

Benzylpenicillin IV 1.2g every four hours plus Gentamicin IV 80 mg BD (monitor levels) . (Click here for duration of therapy - dependent on various factors)

If Gentamicin contraindicated:

Benzylpenicillin IV 1.2g every four hours for 4 weeks if native valve or six weeks if prosthetic heart valve.

Ceftriaxone 2g OD for 4 weeks if native valve or six weeks if prosthetic heart valve.

Reduced sensitivity to penicillin (MIC > 0.1 mg/l)

Benzylpenicillin IV 1.2g every four hours plus Gentamicin IV 80 mg BD (monitor levels), both for four weeks if native valve or six weeks if prosthetic heart valve.
