• Information here on assays/monitoring once daily gentamicin.
• This calculator combines renal function (based on the Cockcroft-Gault formula) ideal and actual body weight to suggest a suitable initial dose for once daily gentamicin suitable in obese and non-obese patients. In non-obese patients actual body weight is used, for obese patients a "dose-determining weight" is calculated. Recommended doses may be given without the usual ceiling doses.
• It should NOT be used for other gentamicin regimens (i.e. BD or TDS dosing regimens).
• This calculator will not produce reliable results in patients who are anuric or in acute renal failure. Advice should be sought from your ward pharmacist or medicines information.
