This calculator combines renal function (based on the Cockcroft-Gault formula) ideal and actual body weight to suggest a suitable initial dose for once daily gentamicin suitable in obese and non-obese patients. In non-obese patients actual body weight is used, for obese patients a "dose-determining weight" is calculated.
It should NOT be used for other gentamicin regimens (i.e. BD or TDS dosing regimens).
This calculator will not produce reliable results in patients who are anuric or in acute renal failure. Advice should be sought from your ward pharmacist or medicines information.
